The Andover Regional School District is sponsoring Kindergarten registration for the upcoming school year. It is necessary for you to make an appointment to register your child.  NO REGISTRATIONS ARE CONDUCTED ON LINE...YOU MUST APPEAR IN PERSON WITH YOUR CHILD.  Registration takes place at the the Florence M. Burd School located at 219 Newton-Sparta Road in Andover Township, New Jersey.  To schedule your appointment, please contact Mrs. Virginia Kaplafka at 973-940-1234, extension 221.

When you arrive for your registration appointment, it will be necessary to have your child with you and to have the following registration requirements in your possession (see list below).  

During the registration process you and your child will need to meet with the school nurse, school secretary and speech teacher.  After the registration process is complete, you will receive correspondence in the mail, usually in July, for Kindergarten visitation to the classroom for late August.

Registration Requirements

To register a child for school, parents/guardians must furnish proof of the following:

•Birth Certificate, or Passport, or Green Card with appropriate signatures and seals

•Proof of Immunization:

  DPT (Diphtheria Pertusis Tetanus)

  4 doses with 1 given after the 4th birthday

  *for students entering 6th grade or after the 10th birthday...TDAP (Tetanus, Diptheria and Acellular Pertussis 

  Vaccine) - 1 dose given after the 10th birthday.  

  Polio Vaccine (Live, oral trivalent)

  3 doses with 1 given after the 4th birthday

  Mumps Vaccine

  1 dose administered on/after 1 st birthday

  Measles Vaccine 

2 doses with the first dose given after the 1st birthday

  Rubella Vaccine (German Measles)

  1 dose administered on/after 1st birthday

  Hepatitis B Vaccine

  3 doses

  Varicella Vaccine

  1 dose given after the 1st birthday 

  Meningococcal Vaccine (for students entering 6th grade)

  1 dose

Also required is a physical examination by a licensed physician for students entering kindergarten or transfer students for whom there is no record of a physical.  This form should be referred to the school BEFORE your child begins school.

•Pupil Transfer Card, if transferring from another district, indicating student's grade level.

•Verification of residence (new registrant) i.e. tax bill, utility bill, lease agreement, etc.
